چکیده

Fractures of the articular surface of the distal end of the humerus (i.e., of the capitellum and the trochlea) are rather uncommon injuries. Fractures of the capitellum account for 1% of all elbow fractures and about 6% of all distal humeral fractures. Fractures of the trochlea are usually encountered as part of more complex intra-articular fractures of the distal end of the humerus, disrupting the medial and/or the lateral column of the humerus. Isolated fractures of the articular surface of the trochlea are very infrequent and are sporadically encountered in the literature as case reports, thus there are only a few clinical series dealing with the description and the surgical treatment of this injury. Trochlear fractures usually accompany elbow dislocations or type IV fractures of the capitellum. Unusual complex injuries about the elbow joint that combine fractures and ligamentous disruptions may be difficult to diagnose and treat. The combination of a ligamentous injury and an intra-articular fracture increases the severity of the injury. Simple elbow dislocations where all ligaments are disrupted have a favorable prognosis when treated conservatively, in contrast to complex dislocations that are combined with fractures and that may require surgical intervention to obtain joint stability. We present an unusual case with concomitant, distinct fractures of the trochlea and the radial neck, accompanied by medial collateral ligament (MCL) disruption, caused probably by a combination of injury mechanisms. Our literature review revealed no other similar case.

Persistent Medial Subluxation of the Ulna with Radiotrochlear Articulation

Two patients-one with a terrible triad fracture dislocation and one with an anterior olecranon fracture dislocation—weretreated for maltracking of the elbow (medial subluxation). The radial head articulated with the lateral trochlea while theulnar trochlear notch was perched over the medial trochlea. The late revision surgery could not correct the subluxationbecause the joints were accustomed t...

Fractures of the capitellum humeri and their associated injuries

Objective Fractures of the capitellum are rare injuries but are often more complex and of a greater extent than assumed from conventional radiographs. Classification is usually based on their extension in relation to the trochlea the trochlea and on the number of fragments. Information on associated injuries is limited and only reported in small case series. The aim of this retrospective study ...

Radial head and neck injuries in children with elbow dislocations: a report of three cases.

In children, traumatic elbow dislocations usually occur after the epiphysis has closed and with associated radial head and neck fractures and osteochondral fragments. The fragments are also usually interposed in the joint restricting complete congruent joint motion. We report on 3 children with traumatic elbow fracture-dislocation with associated radial head and neck injuries treated with open ...

Pediatric Olecranon Fractures Associated With Radial Neck Fractures: Review and Report of Two Cases

INTRODUCTION The debate regarding the description on classification and nomenclature of the injury which includes olecranon fracture associated with radial neck fractures in children is ongoing. We report two pediatric cases that could not be classified in a Monteggia-equivalents system and were treated with open reduction and k-wire fixation. The aim of this study was to perform a systematic r...

Terrible triad of the elbow: a case report of a new variant.

The usual terrible triad of the elbow consists of posterior dislocation of the elbow, radial head fracture and coronoid fracture. We describe a new variant of the terrible triad of the elbow consisting of fracture of the capitellum involving the full length of the trochlea and posterolateral dislocation of the elbow associated with coronoid fracture (type 1 Regan-Morrey). A 25-year-old girl was...
